UVA Health Data Governance
UVA Health is committed to providing a data governance program where data is trusted, understood, accurate and provided and utilized in a meaningful, secure and consistent manner with a community of stakeholders who are trained to utilize appropriately and responsibly.
The submission of a Patient Health Data Governance Form to the Data Governance Council is required if any of the following apply:
• The requestor or data recipient is not a part of our covered entity
• The requestor is sending data outside of the organization
• The purpose of the request is for research
• The vendor received a risk assessment level of HIGH or Undetermined from UVA Health - Health Information Technology – Please include the risk assessment report.
Applies to: All UVA Health team members, School of Medicine, School of Nursing, and other University Partners
Why? To ensure the safety of the patient health information, that disclosures are tracked, and all HIPAA legal requirements are met.
